New Patient Registration Form – Required
This lets us record our pet and owners into our management software so that we can effectively and efficiently provide the service you deserve!
- Payment is expected when services are rendered. In order to focus on our patients’ needs, customer service and minimizing costs, we do not bill.
- We accept debit cards, credit cards (Visa, Mastercard, Discover, American Express) checks and cash. All cards must be signed by the owner of the card.

What to Expect
The Chicago Cat Clinic recommends annual physical examinations and vaccinations as part of our cat wellness care plan. Kittens should be seen monthly in their first four months of life to allow for completion of the kitten series of vaccinations and deworming. We strongly encourage senior and geriatric felines to be examined every 6 months. Ill patients, or those with chronic diseases, should be seen at an interval determined by you and your veterinarian. With each and every visit your cat will be treated with the respect and compassion that it deserves in a quiet and calm environment.
Upon arrival we will ask you to fill out a new client information form. Please bring all previous medical records and lab results to allow us to gain the most information about your feline’s health and vaccination status. Once you are registered in our database we will escort you into an exam room.
In the exam room, a veterinary technician will come and obtain a temperature and weight. A blood pressure measurement will also be done by the technician if your cat is a senior/geriatric cat or has diseases that predispose them to hypertension, such as chronic kidney disease.
The doctor will then come into the room and obtain a medical history from you and perform a full examination. You and your veterinarian can then determine what vaccinations are necessary and discuss any additional testing recommended based on your cat’s age and health status. If your cat is ill, in house testing may be required, possibly including x-rays.
Please allow 30 minutes for a wellness exam. If your cat is ill you may be at the office for 45 to 60 minutes. We want to be able to devote the time that your cat deserves, so please plan accordingly.
