Declining Rabies Form

I understand that Illinois State law requires current rabies vaccinations for all cats. I decline the rabies vaccination at this time. I understand that if my cat bites anyone, including a member of that staff of Chicago Cat Clinic, the cat will need to be admitted for ten days for rabies observation, and rabies vaccination must be given upon release. I understand I will be responsible for all charges incurred for the service.
