Please fill out this form as completely and accurately as possible so we can get to know you and your pet(s) before your visit If this is an emergency of is your pet needs urgent care, please call us at 773-631-5300 for a faster response. Please give us 24 hours to refill a prescription.

If you do not hear from us by the end of the business day, assume your prescription will be ready for pickup within 24 hours of submitting your request. We typically do NOT notify you when your prescription is ready unless you specifically request otherwise.

If you need a prescription filled immediately, CALL US instead of filling out this form. We cannot guarantee that your prescription can be refilled by the end of the business day.
